The Partnership

Formed in late 2001, Aging Well is a partnership of individuals and community organizations in the greater Lyons Township area. The partnership currently includes hundreds of older adults and representatives from over 130 community-based organizations. Partners participate on workgroups and Community Action Teams, provide feedback and input and support the project mission their communities.

Our partnership is continually growing and currently includes:

• elder care attorneys
• retirement planners
• bankers
• community members

And individuals representing:
• assisted living facilities
• local government
• the US Postal Service
• mental health agencies
• social services
• adult day services
• hospices
• nursing homes
• home care services
• transportation services

• local businesses
• paramedics
• park districts
