Goal

This workgroup will focus on the creation and marketing of a single place in the community to call for assistance and comprehensive, accurate information. The workgroup will focus on facilitating communication and service coordination among providers.

The Aging Well Governing Council has approved the recommendation of the Central Access Point workgroup to name Southwest Suburban Center on Aging as the Central Access Point, for information and assistance for older adults and their care providers, in the greater Lyons Township area. Aging Well workgroups, Community Action Teams, partners and local governments have been spreading the word that people should call Southwest Suburban Center on Aging for questions about aging issues. A special “thank you” goes to the La Grange/ La Grange Park Community Action Team for developing the One Call flyer which are now being distributed throughout the greater Lyons Township area.

The workgroup has shifted its focus to facilitating communication and service coordination among providers. At recent meetings, workgroup members have looked at technological solutions to these issues including an electronic referral system that will allow service providers to transmit basic referral information through a secure Web site, track the status of referrals and, with the permission of clients, see a snapshot of services provided to the client by others using the electronic referral system.

Accomplishments

• Named Southwest Suburban Center on Aging as the Aging Well Central Access Point
• Increased community awareness about where to call for information and assistance for older adults and their caregivers, through the efforts of La Grange/ La Grange Park Community Action Team and Aging Well partners
• Identified an e-referral system and will pilot it through a small group of partners
