Goal
This workgroup will focus on the creation and marketing of a single place in the community to call for assistance and comprehensive, accurate information. The workgroup will focus on facilitating communication and service coordination among providers.
The Aging Well Governing Council has approved the recommendation of the Central Access Point workgroup to name Aging Care Connections (formerly Southwest Suburban Center on Aging) as the Central Access Point, for information and assistance for older adults and their care providers, in the greater Lyons Township area. Aging Well workgroups, Community Action Teams, partners and local governments have been spreading the word that people should call Aging Care Connections for questions about aging issues. A special “thank you” goes to the La Grange/ La Grange Park Community Action Team for developing the “One Call” fliers which are now being distributed throughout the greater Lyons Township area.
The workgroup has shifted its focus to facilitating communication and service coordination among providers. The workgroup has secured an e-referral system from the Ann Arbor partnership, made adaptations to it and Aging Care Connections, The British Home and AgeOptions will pilot test it. If it goes well, the group will expand the system to others.
The workgroup is also working to identify a publication to assist the community members in determining the best long-term care services for their needs.
