The Robert Wood Johnson Foundation

The Robert Wood Johnson Foundation provides funding for Aging Well through its national program, Community Partnerships for Older Adults.

Community Partnerships for Older Adults (CPFOA) is a national program funded by the Robert Wood Johnson Foundation to help communities develop leadership, innovative solutions, and options to meet the needs of older adults over the long term.

The Robert Wood Johnson Foundation, based in Princeton, N.J., is the nation's largest philanthropy devoted exclusively to health and health care.  It concentrates its grantmaking in four goal areas: to assure that all Americans have access to quality health care at reasonable cost; to improve the quality of care and support for people with chronic health conditions; to promote healthy communities and lifestyles; and to reduce the personal, social and economic harm caused by substance abuse - tobacco, alcohol and illicit drugs. top of page

The Chicago Community Trust

Community Memorial Foundation

Community Memorial Foundation (CMF) is a private independent foundation dedicated to measurably improving the health of people who live and work in 27 suburban communities west of Chicago.  Health is defined in the broadest of terms to include physical, mental, social and environmental well being.  Its grantmaking focuses on the health needs of infants, children, teenagers, adults and older adults as well as on building the organizational effectiveness in non-profit agencies top of page

Township of Lyons

The Township provides oversight and funding to 17 municipalities in the areas of general assistance, senior transportation, youth, health services, highway commission, special police advisory and mental health commission.  The Township, in collaboration with a many other organizations, provides a broad array of programs and services to residents of all ages.top of page
