Our Vision

Vision

Aging Well is committed to ensuring that older adults are supported in their homes and communities. Seniors and their families should get the help they want and need so that they are able to maintain their independence in the community for as long as possible

Project Goals

Aging Well is implementing an innovative four-year strategic plan to address the priorities identified by the community.  Partnership members formed workgroups and used the information gathered from focus groups, surveys and community forums to propose the following goals and activities.

Access

Ensure older adults and their families have access to community-based supports to maintain independence in the community
• Support a single place to call for information and assistance
• Increase and coordinate transportation options
• Develop an outreach campaign to identify and assist isolated seniors
• Assist older adults with home maintenance and repairs
• Facilitate information sharing and coordination among service providers
• Distribute culturally sensitive, Spanish language information on community resources
• Develop a volunteer senior ambassador program to inform and educate community members about available assistance in greater Lyons Township
• Increase options for short-term respite services top of page

Planning

Increase the quality and quantity of personal planning by educating older adults and baby boomers about financial, legal and care planning options
• Increase knowledge among older adults and baby boomers the wide range of planning options available to them as they age top of page

Direct Care Workforce

Maintain an efficient and effective direct care workforce
• Increase knowledge among service providers about issues that relate to the direct care workforce, such as training, retention and the creation of a career ladder
• Increase job satisfaction and morale among direct care workers top of page

Knowledge

Ensure consumers have the knowledge to make informed long term care decisions
• Improve available resource information so that it is comprehensive, accurate and up to date
• Distribute consumer education materials on ways to assess the quality of long term care services top of page
